Dear Mr.. Holmes,

Thank you for taking time out of your busy schedule to write a brief your brief explanation and alluding to why I should not be concerned about the deceitful facts comprising over 60% of the information in the death certificate.  However I am not going to let my father's death (cause and manner of death) fall through the cracks. 

The matters set forth herein are based on my personal knowledge except where stated on information and belief as to which I beleive to be true. If called and sworn to be a witness I am competent to testify to the truth of the within matters and would so testify.

I understand that by law I am obliged to make this report and request for am immediate inquest, if necessary to exhume the body to determine the cause and manner of death of my father, George Clifford White. If the coroner's office did not see nor pick up the body, there is no legal proof he is even dead. Please confirm. Alot of people have pulled off stunts like this and the pwerson pretened to die to collect death benefits and throw off blood thirsty creditors.(
augusta is invovled in fending off foreclosures on the two residential care facilities, in Marin County.

I am requesting via this email and the facts therein that my father's death was either a homicide I.e. poisons [Digitalis], or unnatural death due to bedsores resulting in urosepsis and/or a contagious disease both a result of fatal neglect. Not to be ommitted as another cause of an unnatural death could be attributed  by the various unscreened foreign workers who Augusta sponsored, from West Africa, employed in the residential homes owned by her
and whom she employed to take care of my father, all of whom could have brought over the AID' s virus and/or other communicable diseases, such as E Bola and could have carried such deadly diseases which ultimately resulted in my father's death.

In addition, the death certificate indicates since there was no attending physician at the time of death which was the reason Dr. Kimmerling could not ascertain as to the progression of the disease. DR. KIMMERLING NEEDS TO BE INVESTIGATED ALONG WITH THE FUNERAL DIRECTOR of McAvoy and O'Hara.

The Law under the Government Code 27941 states:

"A physician usually must sign the death certificate, if the deceased does not have a doctor in attendance who can determine the cause of death, the responsibility for that determination and filling out the certificate falls upon the coroner". 

The death certificate indicates that Dr. Kimmerling last saw my father exactly 72 hours, January 13, three days before his death indicating he was not there at the time of death.

Therefore, I cannot for the life of me understand why, with these facts in hand, that Gary Tindal said in his EMail;

"This death was reported to the Coroner's Office; however, it was released to the Primary Physician for death certification. This office conducted no further investigation".  Gary Tindel, Assistant Coroner 

QUESTION
So are you saying that my father's body was not released to you for identification and other purposes particularly, cause and manner of death, and that your agency knew or should have known about the law of chain of custody? Was my father's body picked up by the Coroner's office? Who was in possession and who delivered the body from Marin County to San Francisco Funeral Home McAvoy and O'Hara clearly, in the jurisdiction of San Franicsco,
 
Even though Ms. Shimano pooh poohs the mistakes on the Death Certificate they are of considerable if not crucial evidence of the major cause of my father's death and shows the intent and mindset of  Augusta White, "informant" to intentionally deceive and misrepresent from beginning to end, including the wrong social security number for my father..IDENTITY THEFT!. Ms. Shimano in her letter tries to cover up these incredibly insignificant mistakes by saying the informant was probably emotionally upset. Uh, huh,
Augusta a professional caretaker in every sense of the word, who deals with these details every day is going to make these kind of mistakes? Not a chance.  These mistakes were deliberately done to deceive. Wake up.

SHORT BACKGROUND

As an intelligent woman, bound by laws and restrictions in the corporate world, I have found that "the devil is always in the details". So let's just back up for a minute regarding my knowledge about the events surrounding my father's life and death..albeit briefly. My 96 year old father met 56 year old Augusta White, while he visited his wife Charlotte G. White-(my stepmother) on a frequent basis at a nursing home in on California Street in
San Francisco around 1992. Augusta was an LVN who lived in the projects at that time and an immigrant from West Africa. I have always been extremely close with my father and was shocked when she cut me off from him.

CRIMINAL ACTIVITY, IDENTITY THEFT, LYING FOR PROFIT, EMBEZZLEMENT, DEATH BY
DESIGN [THEY HAD THE MEANS, MOTIVE AND OPPORTUNITY]

Augusta then became the caretaker to Charlotte in my father's private home in San Francisco. Charlotte within two years died and my father (unbeknownst to me at that time I lived in New York City), at the age of 85, suffering from dementia, married her in a Confidential marriage. (Secret to the public).  My father, a Stanford grad and very successful all of his life was financially secure.  Augusta through the nature of her work had access to financial records, etc. Human nature only dictates the next chain of events.

Suddenly there were huge loans taken on my father's valuable property(s) and at the same time, I was immediately isolated from him. Ms. Augusta made sure to cut off all communications or attempts to see or speak with my father, as she continued to stuff her pockets full of millions of dollars.[Think ANNA NICOLE SMITH]

With that money she moved my father out of the Bay Area and over to Marin to further separate him from his support group[Serenity House and AA group and family] and to sell his home out from under him, with no notice to me or the rest of his children or grandchildren, so that she had total control over
every area of his life. Soon she was bringing in all of her family from West Africa and beyond, to suck him dry financially.  Her efforts were so strenuous to keep him from me that I had to resort to calling the police all the time to do welfare checks on him. Whenever I called he was always "asleep". Was he being drugged?

Augusta's fever to continue to borrow large sums of monies through an illegal power of attorney, signing over properties, via Indenty theft, monies etc., well it doesn't take a rocket scientist to figure out where his life would end up. Augusta was so fearful of me finding out anything about him that she refused to communicate with me despite my efforts to communicate with her..FOR TEN YEARS. Next you are going to ask the obvious questions.."Did you notify the police, did you get a lawyer, the FBI, the DA, The APS", contact the County Recorder's office to tell them about questionable property transactions in Marin? Well, yes I did but those all ended up as allegedly being incompetent, lazy, a continuous shifting of
responsibility, in a dizzying charade of three card Monte with a two card deck.

MOVING ON

Augusta, incredibly bought two residential assisted living homes in 2001, with lonely, elderly, wealthy people.in Marin County and was in charge of the management of those homes. and overseeing the care of the resident/patients Within her scope of duties, she aligned herself with a Hospice Care group who were hired by her and a physician whose specialty was treating diabetic patients, Dr. Kimmerling, of which Augusta suffered from. So this big story that he had been my father's physician for 20 years (everyone has been connected to him for "20 years") give me a break!  BINGO! Her wildest dreams are coming true. her "partner in crime" was Dr.
Kimmerling...you know the doctor you so trust with his conclusion of my father's death? And a San Francisco Doctor at that. If the death fell in your jurisdiction, my father's home for the past eight years was in San Rafael, why wasn't a San Rafael Doctor ever part of his healthcare treatment plan?

Answer - Because Augusta And Kimmerling are a team to defraud, kill, and collect the spoils during and after a person's life. This doctor on many occasions has been belligerant, mean and cruel to me, whenever I tried to find out how my father was. He played "hide the ball" with me, yet despite my telephone calls and written requests to Augusta and him, surrounding my father's health status, they refused to respond and treated me with gross disrespect.

By law you are also supposed to notify me or make an attempt, as well as stated within in the California Government Code 27941.

In case you have a screw loose and want to run to their side and excuse their behavior. Think of this...besides my numerous facts with the death certificate, pointing out the glaring errors which were obviously stated to DECEIVE AND DEFLECT. why would Augusta fail to file the will for probate within THE THIRTY DAYS ALLOTTED FROM DATE OF DEATH, which is required by law? Why would she refuse to open a probate file at all?  Why would she refuse to notify me and the GRANDCHILDREN of their grandfather's death?

Either way I am the victim here and am suffering  - just the very act of having to go through writing these Emails, reading and processing them as well as dealing with the shock of this untimely and unexpected rash of surprises, I have bee damaged because as a CEO of a Media conglomerate, this situation requires immediate attention, with the necessary gathering of the facts before I come to a conclusion to turn it over to the appropriate people, my business has suffered and will continue to do so until I am given the exact truth of the cause and manner of my father's death even if it
requires the exhumation of his body.

I did not create this situation, you and others, by mistake, oversight or laziness have and now I am requesting aggressive action to get to the bottom of this because you, Mr.. Holmes are in charge and responsible for the investigation of the real cause of death and the manner.

Common sense dictates that a person's death does have steps leading to it. You cannot cavalierly say, it was due to age!  I want a set by step progression of his disease, time and death of all those pieces of the puzzle.  I want an independent death investigator to be assigned who cannot be bribed!

Oh, by the way, my attorney found the obituary yesterday and sent it to me. Of course there were lies and ommissions sprinkled thorugh it causing me even more pain. Neither my brother or our children or me were mentioned by name and place of residence, she knew where we lived, she just wanted to pretend she d idn't in case, maybe if she had to swear in a court of law that she didn't and backed it up with the obit. I was suprised the "guestbook" only had foru entries when the funeral director told me i The lack of notification of me concerning my father's death has caused me
such grief and pain I cannot begin to tell you and now i have to go through all this because nobody did their jobs.  I had to do the job for them.  Now I want immediate action as you will have to swallow your pride and do the right thing for me, my father and his grandchildren.

I am closing with this article which essentially states that you Mr. Holmes have the power to say whether this will proceed to finally determine the cause of death of my father or if he even died. Considering the Grand Jury Investigation in 2001 regarding specific updates you need to make and with regards to concentrating on the cause and manner of death issues, I would, if I were you and interested in your standing in the community, get to the bottom of this immediately and become a leader in your profession. other
wise you may have to go on the junk heap of the rest of the politically inclined voted in by the public.

    Gary Tindel and I have discussed your email and I would like to add some information beyond what Ms. Shimano has provided for you. I will address that portion of the death certificate below the Cause of Death section because Ms. Shimano gave you the exactly correct explanations for the upper portion of this legal document.  Understanding government forms can certainly be a test however I hope that if I can help you understand what this form is expected to do you will have a clearer picture of why the limited information is asked for and what it is expected to accomplish.

    First, please understand that the official death certificate in the state of California is not meant to reflect or comment on any treatment, medical or otherwise, that any person has received during the course of their life. The fact that your father was previously treated for some condition, and the medications he was prescribed is NOT something that is ever addressed on a
certificate of death.
 
Dr. Kimmerling is required to provide the primary cause of death and any underlying contributing conditions.  He is also restricted in the amount of information he is allowed to list.  He did, in fact, report your dad's death to my office.  The "Coroner's referral number" listed to the right of the primary cause of death is a number provided ONLY by my office when a death is reported.  The "Time between onset and death" is an estimate unless an exact moment is identifiable. ( ie; the exact time of a car accident, or
known time of an earthquake, etc.)  I typically use terms like: "hours" , "days", or "weeks" because most causes of death cannot be ascribed to a specific moment.  ( ie; when did the first cancer cells begin to grow ? )
 
Box 112. :  Senile dementia is listed precisely because it is a neurological condition that affects the brain cells.  In doing so it severely limits the body's capability to react properly to other problems as they arise.    The dementia did not cause the condition in line 107, but because of the dementia your father was somewhat compromised and the condition was able to progress.  THAT is significant in the reporting of what happened to lead up to #107.

    Epidemiologists derive mountains of information from this form.  The certificate is not, however, intended to detail each and every moment of one's medical history.  That information is found in medical charts, records and histories.  By virtue of it's size alone this document cannot be expected to tell one's life story, only the direct cause(s) for the cessation of that life.
 
Manner of Death:  This section of the death certificate can ONLY be filled out by the Coroner.  Our California Government Code dictates that a physician of record can only sign a death certificate if the death is the result of "natural causes."   Therefore the presumption is that if Dr. Kimmerling's signature is on the certificate in the first place, the death must then, by definition, be from natural causes.  If a death is the result of any other "manner" :  (accident, suicide or homicide) a personal physician is forbidden from signing the document and the Coroner MUST then conduct an investigation, issue and sign the certificate himself.  That is part of the "checks and balances" created by our state constitution.
 
In conclusion:  the medical portion of your father's certificate is correct and complete.  The information which is incorrect in the "personal data" portion of the certificate will be changed at the state office of Vital Statistics after you have completed and mailed the amendment form which Ms. Shimano was so kind to provide to you.
 
I sincerely hope this information will help you understand at least the "red tape" portion of what happens when someone dies in California. If  you have further questions I will be happy to answer them.
