Dead Men Do Talk: The Death certificate and its inconsistencies and the rush to cover it up by the coroner, the Funeral Home and the County Recorder are revealed on this page. Just the facts..our most reliable witness. You be the judge.
Dear Dr. Schwartz, Ms. Thayer, Ms. Maldinado and Ms. Collins and Ms. Tindel,
I am the biological daughter of Decedent, George Clifford White, who passed away January 16, 2007 in San Rafael, in Marin County.. Please see the attached Death Certificate.
Upon receipt of this certificate, I noted several discrepancies, and will start from the beginning of the Certificate with supporting facts. The errors are the following:
#3 AKA's - This was left blank, however throughout the entire relationship between my father and Augusta, and the numerous financial transactions, he had several AKA's. Please refer to attachments Exhibit's A. FYI, The Funeral home lists my father's ID as George C. White, not George Clifford White.
#8. The hour of death was not represented as am or pm
#9. Bachelor is clearly an error. Please see attachment Exhibit B as it relates to my father's education at Stanford University,
#19. My father was a published author and a consultant right up until dementia started around age 80. So the correct number would be 60 years not 36
#37. My Grandmother's name was O'Hoolihan not Hulahan
#46. "Signature" of local registrar does not match with the true identity of Annie Maldonado, or Ms. Collins, CO-Deputy Registrar N. San Pedro Road, Suite 2006San Rafael, CA 94903 Office (415) 499-6876
Part 1 CAUSE OF DEATH*******
A. EUROSEPSIS may have resulted in my father's death, "etiology unknown" is an untrue statement. Everybody knows that this is a bacterial infection caused by any number of conditions (E-Coli-Pneumonia, etc.) and mostly cured via antibiotics. That was what was administered and treated successfully at Marin General in August 2006 at Marin General. In other words, if this event hadn't happened would decedent still be dead?
Time limit between onset and death 36 hours This does not make sense
B ANTACEDENT (older) George Kimmerling, MD of San Francisco, omitted cause of death due to the consequence of ANTACEDAENT (older) Condition resulting from. My father was treated, hospitalized and released after one week in August 2006 for the same diagnosis as outlined in Exhibit B.
C. George Kimmerling, MD omitted what the First (oldest, original) Condition
D. George Kimmerling, MD omitted the Underlying Initiating Event
Part II OTHER SIGNIFICANT CONDITIONS - Conditions contributing, but not resulting in the underlying cause of death
"Senile Dementia" was listed. This is a neurological condition, causing degeneration of brain cells. What does this have to do with my father's death? This does not focus on the specific fatal event. This also does not assign to the pathological or environmental processes of events to death, which prevents pertinent
information for epidemiologists who rely on these certificates.
MANNER OF DEATH
Circumstances of Death - was omitted and not completed even though 90 days have passed since my father passed away. The Certificate states the Coroner was contacted. Was he?
Physicians like George Kimmerling, MD only certify natural deaths, I find it quite unusual this would be omitted.

In Summary: This Certificate, on its face represents untrue information and omits the necessary Death Certificate requirements, thereby making it void. In addition, his name on the Death Certificate reads George Clifford White, yet the funeral home and cemetery list him as George C. White. My question is who really died? Was it George Clifford White, George White, or George C.
White?
Therefore, I am requesting an immediate investigation be opened with errors corrected and a more definitive cause of death be determined along with the completion of the omitted and necessary certifications required by California State Law.
I find it hard to believe that Dr. Kimmerling who states he has been my father's Primary Care Physician for the last eighteen years, would be unable to fill out this Certificate in a more cogent, illustrative manner.
As a side note, and the major reason I am contacting you is I was never notified of my father's death, nor invited, to the funeral, even though the various parties involved in the death certification process had my contact info, nor was any announcement made via an obituary. It was a cruel and calculated action.
Living in Southern California, I could have certainly taken a 45 minute plane flight to San Francisco to be at the viewing and vigil the night before the funeral and the funeral services as well.
These unanswered questions I have concerning my father's death are necessary in the processing of both the shock of discovering his death only a few days ago (90) days after his death, and the necessary process of grieving and ultimately the closure I need. To say I am devastated is an understatement.
Thank you for your prompt attention to this disturbing, urgent matter.
